Can't we all just get along? Achieving successful benefits integration with multiple carriers
Today, effective benefit design considers not only costs on budget lines but costs to human capital. More than ever, employers are interested in health and productivity data and what that data means horizontally across integrated benefits. "The plans are a source of very rich data and it has the potential to sew all the pieces together, even if the employer doesn't buy all products from one carrier," says Dennis White, senior vice president of Value Based Purchasing at the National Business Coalition on Health.
Collaboration among benefit carriers allows areas like health promotion, disease management, absence management, prescription drug coverage and health insurance to share data and align with the employer's goals. Integrating benefits can help each plan do their job better, but getting carriers - who are often competitors - to work together and share information can be a major undertaking.
"The wellness carrier should have lab results, pharmaceutical records, claim information, health risk assessment data, etc.," says White. "But usually, neither the plan nor the company are organized enough to give that information."
Bring them to the table
"Get everybody in the room at the same table. That's the first and most important step in integrating benefits," says Thomas Parry, president of Integrated Benefits Institute. "This is where employers and their vendor partners can really work together as genuine partners."
While employers shouldn't launch an integration initiative without setting concrete objectives for the change, they also should keep an open mind as carriers assess what areas they can share information and better serve employers.
"The mid-sized and larger employers are saying, If you can't work as a partner with all of us, than you're not a good partner,'" says Parry.
Holly Snyder, president of Nationwide Better Health, a carrier that offers several integrated health care services, says employers often will purchase couplings of their products - like disease management with health promotion - and not buy other services in their package. When sitting down with other vendors in the health management area it is important that employers single out a "lead vendor" who can help integrate all the carriers together, she says.
"Look for someone with a little more experience in the field and someone who has a spirit of collaboration," says Snyder. "If you don't really define the integrator or lead role, then the responsibility will fall on the employer, which - depending on their expertise - could be a problem."
Another important stakeholder to have at the table is the CFO, White states. An HR/benefits executive who engages her CFO in the conversation will be better prepared to pinpoint the key issues they want carriers to focus on. CFO involvement can be critical to integration because many companies have separate budgets and managers to accompany each benefit silo.
"This causes financial and management barriers - everyone carves out their own solution not realizing how to sew them together," says White.
Not only should employers combine management and budget sections, they also should locate the main human capital issues across benefits. When the focus has been established, carriers can decide what data is required to address the company's concerns, says Parry.
Set standards, expectations
What each carrier is responsible for must be determined up front and collaboratively. "Make sure that you are aligning goals so people aren't working at cross- purposes," Parry says. For example, an employer can't hold a health insurance plan accountable for only minimizing costs and hold an absence management carrier accountable for only decreasing absences because the two goals are not necessarily compatible.
Like in parenting, expectations are very important to getting outcomes, says Parry.
"Some companies are trying to note where the deficiencies are and then put some teeth in it with financial consequences and performance guarantees," says White.
Also, to collect and share workforce data, all carriers must have standardized data language and metrics. "When two insurers look at duration metrics, are they looking at the same thing?" says Parry.
"Virtually no one has all the data they want and that the employer wants," Parry admits. "It doesn't happen fast or without cost, so a timeline is key as to when certain data should be collected."
Extra credit
The saying, "You catch more flies with honey than vinegar" holds true in multivendor integration efforts, the experts contend. Carriers should be rewarded appropriately for working together to meet expectations. But much like a group project in school, how does an employer know which carriers on the team were working hard and which members were slacking off?
"There should be tight contractual language and measurement systems to make sure they each understand their role and get the credit they deserve," White advises.
Adds Snyder, "Consultants are really interested in this and have formed different analysis methods to divvy up the credit."
Is integration a myth?
It seems obvious that getting multiple carriers to be team players when they want to stand out to their client can be a struggle. From a carrier viewpoint, Snyder says "It's getting there. Whenever there is enhanced collaboration across many vendors it is a major improvement."
Integrating data and analyzing data to a point where it is useful can be extremely challenging for an employer. Integrated data warehouses, consultants and third-party administrators are more than happy to help all parties collaborate - and business is good.
"The data warehouses can attest: Employers see this is the direction they need to be going," says Parry. - M.B.
